The City provides the following benefits to eligible full-time and part-time employees through its Civilian LAwell Benefits program (“LAwell”):

Health Insurance – Effective for 2017, City employees may choose from five different plans: (1) Kaiser Permanente HMO; (2) Anthem Narrow Network (Select HMO); (3) Anthem Full Network (Ca Care); (4) Anthem Preferred Provider Organization (PPO); and – New for 2017 – (5) Anthem Vivity (LA & Orange Counties). The HMO options provide healthcare through a network of doctors, hospitals, and healthcare providers, with access available as directed by your Primary Care Physician. Residence/worksite proximity may apply. The PPO option makes nationwide networks of doctors, hospitals, and other healthcare providers available both inside and outside of a healthcare network, with the latter generally being at higher cost to the member.

Vision Insurance – Effective for 2017, City employees will receive vision care benefits through a single vision plan, offered through EyeMed, at no cost.

Dental Insurance – City employees may choose from three different plans: (1) DeltaCare USA HMO; (2) Delta Dental PPO; and (3) Delta Dental Preventive Only. The HMO option provides dental care through a network of dentists; the PPO option makes dental care available both inside and outside of a dental care network; the preventive-only option only covers preventive services.

Life Insurance – Provides a base policy with the option to purchase higher levels of coverage.

Disability Insurance – Provides a base policy with the option to purchase higher levels of coverage.

Accidental Death and Dismemberment – Provides option to purchase this coverage.

Healthcare Flexible Spending and Dependent Care Reimbursement Accounts – Provides a tax-savings option to set aside money for eligible healthcare and day care expenses. Money set aside in these accounts must be used in the calendar year and does not rollover.

Employee Assistance Program (EAP ) – Provides counseling services at no cost to the employee.

LAwell Benefits provided to eligible employees make up a valuable part of employees’ total compensation. A major portion of the LAwell benefit costs are paid by the City’s subsidy. Employees may pay some portion of LAwell benefit premium costs based on the provisions of their labor-negotiated Memorandums of Understanding (MOUs) and the level of coverage they select. In addition, a member may be required to pay doctor visits, prescription drugs, and hospital copays when seeking services. An employee may also opt out of health coverage entirely and receive up to $100 per month of “cash- in-lieu” of medical coverage. Sworn Firefighters and Police Officers may elect to have their benefits provided by LAwell Benefits or other plans specified in their MOU. Employees of the Department of Water and Power (DWP) have their own benefit programs and may not opt into the LAwell Plan.

Q. How Do I enroll in LAwell Benefits?

Q. When do I obtain my benefits?
A. As a New Hire, 4 to 6 weeks after your first day on the payroll, a LAwell Benefits Enrollment package will be mailed to your address listed in the payroll system. You must make your elections within 60 days of the date on your Personal Enrollment Worksheet. Your benefits will become effective the date you call or enroll online, however it may take 4-6 weeks for your information to be completely processed by all affected insurance providers. You may visit your doctor after enrollment, but it is recommended that you have your provider verify your coverage prior to obtaining services.

Q. When can I make changes to my benefits?
A. Generally speaking, employees are able to make changes to benefits election during an Open Enrollment period which occurs in October of each year (for coverage effective January 1 of the subsequent year). Employees are also able to make changes if they experience a qualifying Life Event Change. If you have a qualifying Life Event Change, you must call the LAwell Benefits Service Center (800) 778-2133 or go online within 30 calendar days of the Life Event Change to report the changes and make new benefit choices and supporting documentation may be required within 60 days.

Q. What is a Life Event Change?
A. A Life Event Change can be marriage, beginning or ending a domestic partnership, birth, adoption, divorce, death, addition or loss of an eligible dependent, or a loss or gain of spouse/domestic partner health coverage. You must report a Life Event Change within 30 days of its occurrence and submit documentation within 60 days in order for the change to take effect.

A. You may cover your spouse or domestic partner, child(ren), and grandchildren if they meet the LAwell dependent eligibility criteria and you submit the required documentation by the deadline. Q. How do I change my mailing address?
Contact your payroll or personnel office if your address has changed; this will ensure that your address is updated with the LAwell third-party-administrator and you receive notices and updates.
