The Personal History Statement is part of the background investigation portion of the Selection Process. You are not required to complete this form unless you have been scheduled for an initial background appointment. However, this form is very extensive and requires that you obtain several documents not readily available to you. Because you need to fill out this form in time for your initial background, it is provided online so that you may begin working on it at any point.
